Family Support: How to Apply
Who We Serve
If your child has been diagnosed with a life-threatening critical medical condition, your whole world has just been turned upside down. The Mitchell Thorp Foundation can help guide you through the maze of paperwork, provide an emotional foundation, advocated on your behalf, and give you the tools and resources to cope with the extraordinary demands of dealing day-by-day with your child's and your family's needs. The foundation provides culturally competent, trauma-informed care and equitable access to all services.
Upon acceptance as one of Mitchell Thorp Foundation's client families, you will have the choice to opt-in for a free crowdfunding page to raise money for your child. All money raised goes directly to vendors of your services and there are no taxes or fees. Please complete and submit photos of your family, along with 1st two pages of last year’s tax returns and a few photos of your children, one with the child going through treatment and a family photo.

Applying for Support
Applying for support may feel overwhelming, but we’ve worked hard to make the process simple and respectful. Every application is reviewed with care by our team, and all information remains confidential. Our goal is to ease your burden, not add to it. If your child is facing a life-threatening medical condition, we invite you to take the first step—because no family should walk this road alone.
Submit the Application
Complete and send us the official application along with proof of income and the first two pages of last year’s tax return, and submit photo of child going through treatment and a photo of the whole family.
Support Assessment
Our Board reviews applications weekly. We evaluate each case individually and contact you as soon as an enrollment decision is made.
Personalized Assistance Begins
Upon approval, we work with families and vendors directly—setting up payments for your needs while preserving your benefits and peace of mind.
Optional: Crowdfunding Support
You can opt-in to receive a free, tax-free crowdfunding page on our platform—proceeds go straight to vendors and come with zero fees.
